Dear Parent/Guardian,
Akron Childrens Hospital- School Health Services is working with your child’s school to give the annual influenza (Flu) vaccine to children. Flu clinics will be offered at school during school hours. Participation is voluntary and available to ALL consented students, regardless of their insurance coverage. Please fill out the consent form by November 9, 2023 for your child to participate in the vaccine clinic.

Why get a flu vaccine?
The best way to minimize or avoid the spread of flu is to get an annual flu vaccination. Each year the vaccine protects against new strain. Although student participation is voluntary, flu vaccines are recommended every year to help keep kids healthy and in school.
What type of flu vaccine will be offered at my child’s school?
Vaccines will be offered to all consented children who do not have any contra-indicators. All vaccines given are preservative-free, mercury-free, Thimerosal-free and administered by state-licensed nurses.
How can my child participate?
Review the vaccine information sheet (VIS) and return the attached completed consent form by November 9, 2023.
